                                                                              ACKNOWLEDGEMENT

By applying for a Science Academy program, Parent(s) and Legal Guardian(s), understand and agree to all the following. Parent(s) and Legal Guardian(s) understand and agree that enrollment to any of The Science Academy’s programs is on a first-come, first-served basis and subject to limited available spots. No registration can be considered a completed registration until all payments are fully received, processed, and cleared up by the Accounts Receivable Department. You also understand and agree that due to fixed costs and expenditures that are based on definite enrollment, no refunds, reductions, or credits can be issued. Parent(s) and Legal Guardians also understand and agree that if you request a refund or credit, The Science Academy might consider a refund or credit based on certain circumstances and may require paperwork to back up those circumstances. If approved for a credit or refund, it will be for 50% of the unused tuition balance, plus a $25.00 processing fee. Registration Fees are non-refundable. Please note, if requesting a refund or credit, there are no guarantees for refunds or credits. You also understand and agree that if you pay by check and your check payment is returned for any reason, you agree to pay a $25.00 returned check fee plus any collection costs incurred by the Science Academy.

Also, Parent(s) and Legal Guardian(s) understand and agree that all students must have Fine Motor Skills to attend and be able to complete any of the Science Academy’s programs. Any student that does not have Fine Motor Skills may be dismissed from all programs at the Science Academy and no refunds or credits will be issued.

Parent(s) and Legal Guardian(s) understand and agree, that to provide a high-quality experience for all students, we maintain a positive and focused learning and fun environment. If a student’s behavior becomes consistently disruptive, the instructor will work with the student to correct the behavior. If the disruption continues, parents will be notified to help find a solution. The Science Academy reserves the right to dismiss any student from the program, without a refund or credit, if their behavior significantly interferes with the fun environment, safety or learning of others. Safety is our top priority. Students are expected to follow all instructors’ directions and respect their peers. Serious disruptions, bullying, or unsafe behavior will result in immediate removal from the program.

Please note, our facility is not licensed as a diaper-changing environment, and our staffing ratios do not allow for the 1-on-1 time required for toilet assistance. For the health and safety of all students, we must adhere strictly to our potty-trained policy. Parent(s) and Legal Guardian(s) understand and agree that all Students must be fully potty trained before the first day of the program. We define "fully potty trained" as the child's ability to: Identify and communicate the need to use the bathroom in advance. Doffs and dons clothing without assistance. Manage all aspects of toileting independently (wiping and flushing).  Note: Pull-ups or training pants are not permitted. If a child has frequent accidents, we may ask that they withdraw from the program until they are fully potty-trained.

Parents and legal guardians understand and agree to complete the Emergency Contact and Medical Form and disclose any student medical conditions to the Science Academy Directors before the program begins. Failure to do so can lead to dismissal from the program. This also includes any medication the student(s) may be taking. Parent(s) or Legal Guardian(s) understand and agree that The Science Academy and its staff are not responsible for administrating any type of medication.

Policy on External Support Providers. The Science Academy is committed to providing a safe and inclusive environment for all students. However, due to licensing, liability, insurance, and safety regulations, we are unable to allow outside therapists, shadows, or behavioral consultants on-site during program hours. Our facility is licensed exclusively for our trained staff and registered participants. This ensures that every adult in the building has been vetted through our specific background checks and safety protocols. We have a strict duty to protect the privacy of every student in the room. Having a non-staff adult observing the class, even if they are there for only one child, compromises the confidentiality of the other students’ behaviors and learning progress. Also, in a classroom setting, it must be clear to the students who is in charge. Having an outside adult giving different instructions can confuse the child and undermine the teacher's ability to manage the group safely. Our classrooms are optimized for specific student-to-teacher ratios. Adding additional adults creates overcrowding and can hinder the 'flow' of the hands-on science experiments we conduct.

Parent(s) and Legal Guardian(s) understand and agree that The Science Academy establishes communication channels for parents to stay informed. By providing your mobile phone number and email address on the application, you consent and agree to receive text and email messages from the Science Academy regarding your Registration, Class Dismissal Procedures, School Alerts, Program Updates, Promotional Messages, Sponsorships Information, and more. We only share data with trusted service providers who help us operate our business (e.g., payment processors, email delivery services, and hosting providers). These partners are strictly bound to keep your data confidential.

4. Cookies and Tracking: We use cookies to analyze site traffic and remember your preferences. You can manage or disable cookies through your browser settings, though some site features may not function properly without them.

5. SMS Privacy: No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. All the above categories exclude text messaging originator opt-in data and consent; this information will not be shared with any third parties.

6. Your Data Rights & Choices: Depending on your location, you have the right to access, update, or request the deletion of your personal information. You can opt out of messages anytime by “Text STOP to opt out, HELP for help. Or unsubscribe from emails. 

7. Should a situation arise from a student’s negative behavior in the program (Class disruption, Bullying, disrespect of their peers, unsafe behavior, etc.) and an incident report was established, the names of students in this incident and report will be kept private. Only to be shared with the Parents or Legal Guardians of the Student that created the incident, and Staff.

Parent(s) and Legal Guardian(s) understand, agree, and give permission to The Science Academy so it may use images of student(s) and the work created by student(s), for the purpose of promoting and recruiting for The Science Academy programs.

Acts Beyond Our Control: Parent(s) or Legal Guardian(s) understand and agree that if any circumstance arise such as war, tornado, hurricane, government restriction, acts of God, epidemic, pandemic, power outages or any other circumstance that the Director of The Science Academy finds it advisable to close or not open the Program, no refund or credit will be made. 

Parent(s) or legal guardian(s) understand and agree, that The Science Academy is not responsible for any possible exposure to any type of illness or infectious diseases including but not limited to influenza, MRSA, COVID-19 or any illness or infectious diseases. For the safety of the students and staff, the parent(s) / legal guardian(s) named in the application also acknowledge and agree that if their child or children have encountered anyone showing any symptoms to illness, they will communicate this information to The Science Academy’s staff and that the child or children will stay at home. Parent(s) or legal guardian(s) named in the application also acknowledge and agree to accept all responsibilities if they fail to follow this agreement. Parent(s) or legal guardian(s) named in the application also understand and agree, while safety precautions and personal discipline may reduce the risk of illness, a risk still does exist. As a parent or legal guardian, you knowingly and freely assume all such risks, both known and unknown and assume full responsibility for your child or children's participation. Parent(s) or Legal guardian(s) named in the application and their child or children, hereby release and hold harmless The Science Academy, their employees, sub-contractors, owners, partners, and all other participants with respect to any illnesses or any accidents whatsoever.

Please note, The Science Academy is not affiliated with any school district or any schools in any district. The Programs, Events, and activities hosted by The Science Academy on any school premise pursuant to a lease agreement shall not be construed as being conducted, funded, hosted, or sponsored by the school board or school on behalf of the Science Academy. The school board and its schools have no responsibilities for supervising or monitoring The Science Academy programs, events, or activities and cannot be liable for any actions of the Science Academy at any school premises. Also, The Science Academy cannot be liable for the School Board or School's events, activities, or conduct of any of their personnel at any location that The Science Academy is conducting their programs. We at The Science Academy hope that all parents understand that all our programs and activities are developed and executed by the Science Academy's staff and STEM Education partners and not by any school boards of any county. We utilize real world STEM Professional contributions from Scientists, Physicists, Mathematicians, Engineers, Business Executives, etc. from several STEM related backgrounds and organizations to help produce our programs and activities. 

The Science Academy maintains high standards for program integrity and campus safety for all participants. As such, Parent(s) and Legal Guardian(s) also acknowledge and understand that a refusal to agree to these terms, in this Acknowledgement, the Science Academy will have the right to void or cancel any application or registration at any time with no refund or credit.
